ERP Q/A Call Descriptions

  • Application & Annual Status Report Q/A Call: Questions concerning the application process for Original and Renewing programs services, Additional Site applications and the Annual Status Report.

  • New Quality Coordinator Q/A Call: During the call the criteria for the 10 National Standards for Diabetes Self-Management Education and Support will be reviewed as well as the ERP website, resources and templates. Each participant will have the opportunity to ask questions during the call as well.

  • Krames On-Demand Walkthrough Q/A: A walkthrough of the Free Krames Multi-Language Patient Education materials available to you as a Recognized ADA DSMES Service Resources include the Living Well with Diabetes booklet, Living with Type 2 Diabetes Program and Krames On-Demand Non-Branded patient education materials.

  • Chronicle Diabetes Q/A Call & Webinar: A webinar and walkthrough for Chronicle Diabetes. Participants will have the opportunity to ask questions and network with other attendees.

  • DPP Express Q/A Call & Webinar: Chronicle Diabetes, diabetes education and outcome tracking HIPPA and HI Tech compliant electronic charting system is being expanded to capture DPP data and sessions. Each participant will have the opportunity to ask questions during the call.
