Make a Memorial Donation

Creating a living memorial in the name of a family member or friend is the perfect honor in support of the American Diabetes Association. Donations to the American Diabetes Association help fund leading-edge research impacting the health and well-being of millions of people living with diabetes. Your gift is put to work immediately to support important research efforts and help provide much needed services to people with diabetes.

Your tax-deductible donation shows the family of the deceased that you care about their loved one while helping the American Diabetes Association fund research to find a cure for diabetes. We'll send an email, that you personalize via Ecard, or mail a card personalized with your message to the person you designate to notify of your donation (the amount of the gift will not be indicated). If you'd rather, you can make your donation by phone (1-800-DIABETES) or mail.

• Ecard/Email Acknowledgment

  Choose this option to acknowledge your memorial gift with a personalized Ecard by email. This option saves ADA the cost of postage and printing so that more of your donation can go directly towards diabetes research and programs. Thank you. Select Ecard/Email Acknowledgment.

• Printed/Mailed Acknowledgment

  Choose this option to have ADA send a printed card for delivery by mail. We will capture your information and send a printed card as notification of your memorial gift. Select Printed/Mailed Acknowledgment.

• No Acknowledgment Card

  Choose this option if you'd prefer to send no card at all.
